Nurse Online Application – User Guide

1. Introduction:

 

     This user guide describes how to use IQMAN Nurse Online Application, a web application that can be use by the nurses that wish to apply to IQMAN. In this guide, we cover:

 

  • How to register as a new Nurse Applicant and create an account.
  • How to login to IQMAN Online.
  • How to edit the data that you put into the registration form.

 

2. Getting started with IQMAN Nurse Online Application:

    

If you are a new user of IQMAN Online, you have to create a user account. But before that, we remind you the following pointers:

 

  1. Do not use single quotes ( ).
    • This is prohibited because the usage of single quotes causes error in the program. You may use other characters in lieu of using single quote (e.g. ` ).

 

  1. All dates must be in this format “MM/DD/YYYY”  (except for those in Prof. Experience)
    • In order for our data to be consistent, we advice everyone to use the said format for all the dates to be inputted in the system.

 

  1. Exam dates must be included and is in the proper format (MM/DD/YYYY)
    • You must include the exam dates of your exam because it is the only way we could verify if your exam is not expired and this is also used in computing for your exam ranking and overall ranking.

 

Given all of these, you can now use the IQMAN Nurse Online Application.

 

How to create a user account:

 

a.      Open your internet browser and type “http://www.iqteam.comon the address bar.

b.      Click the “IQMAN Nurse Online Application” on the Client tools portion in the lower leftmost part of the page.

c.      Click the “create an account” on the left side of the homepage.

 

 

d.      Fill out the registration form for new user. Make sure that you fill out all the required fields that are marked with red asterisk (*) those are important information needed.

 

Required fields for Nurse Registration Form        

       

  • Firstname, Lastname, Middlename
  • Date of birth: The format should be mm/dd/yyyy.
  • Contact Numbers
  • Available Schedule for Interview
  • Password: must be at least 6 characters only.
  • Branch Email: If you want to know its email address just select an IQMAN Branch Nearest you and the branch e-mail will appear.
  • Degree and School for College (in Educational Background)

 

Please bear in mind that as long as you have completed all the required fields, you may submit the form to expedite the creation of your account and then login to continue to fill up the other information.

 

              

 

In the Educational Background just fill out all the information needed. The required fields are the degree and the school of your college education background. Just like the figure below.

 

          And for the next part which is Professional Experience, you may skip this portion to expedite your application since this is not yet required to be filled up. This is recommended so as to avoid submitting too large amount of data. You may return to this portion again if you successfully registering to the system and logging on to the system. However, you may still fill up this portion up if you want.

 

    

     And for the last part which is Examinations Passed, you may skip this portion like in Professional Experience part, in order for you to expedite your application since this is not yet required to be filled up. Again, this is recommended so as to avoid submitting too large amount of data. You may return to this portion again if you successfully registering to the system and logging on to the system. However, you can still fill up this portion up if you want. Just mark the box provided with a checkmark to select the corresponding exam that you have passed.  

 

 

Again we remind you that you can complete registering by just filling up all the required fields. Once you are done, click “Submit” button to register.

 

 

3. How to login to the IQMAN Online

 

         Now that you have successfully registered, you may now login to the system. An e-mail containing your login information will be sent to the e-mail address you’ve indicated in the registration form.

 

a.      Open your internet browser and type “http://www.iqteam.comon the address bar.

 

b.      Click the “IQMAN Nurse Online Application” on the Client tools link in the lower leftmost part of the page.

 

c.      Click the “IQMAN Nurse Online Application”.

 

d.      Login by clicking the “Login to IQMAN Online” or you may click “LOGIN” in the menu bar at the top rightmost portion of the page.

 

e.      On the Home page of IQMAN Nurse Online Application, choose the “Login to IQMAN Online” link where you can login as a new user. You'll be taken to the Main Menu page.

 

 

 

 

 

The username that you will enter is the combination of your Last name and your Birth Date. (e.g. bernardo063086) just remove the slash on your birth date which you’ve entered in the registration form and as well as your Password. (See example below)

           

How to Edit Your Application Details:

 

  1. Check your “application status” at the top right corner of the Home page if it's active or not.

 

 

 

 

  1. If it's active or selected, you cannot edit any data. If you want to edit or you want to change the error (if there's any error on the data and you want to change it), contact anyone of the IQMAN employees what you want to change on your data then they are the one to change it. If not, then you can edit the information that you put or add other data by clicking first the category in the menu in the leftmost portion of the page.

 

  1.  The Nurse Online Application is divided into 12 parts and below are the detailed explanation on how to input / modify / delete information in each part.

 

I. Personal Details

 

  1. Now that you are not an active/selected nurse yet, you can still modify your application details.
  2. If you wish to edit your Personal Information, just click the Personal Details in the menu bar.
  3. In this portion, you may also add your Place of Birth and Present Address or even upload your picture. Just click the Save button to save the changes you made.

 

 

 

II. Educational Background

 

  • Next one is Educational Background, if you wish to edit this. Just click the item on the menu.
  • If you want to add a new educational background, enter the details first and just click Add Education button if you are done.
  • If you wish to edit an existing record, just click Edt on the Options part. The details would be loaded and you may now edit the corresponding details you want to modify and click Update Education to save your changes.
  • Meanwhile if want to delete an existing record, just click Del on the Options part.

 

 

 

III. Professional Experience

 

  1. We move on to the next part which is the Professional Experience. If there is no Work Type selected, all items are disabled.

 

 

 

 

 

  1. To edit an existing record, just look at the bottom part of the page and see if there are already existing records. If there are existing records, select the specific record and click Edt. This results to the loading of the information for that specific experience. Also, you may also delete a specific experience by clicking Del. Otherwise, if you are not editing or deleting, fill up all the necessary information and just click Add Experience button to include that experience to the list.

 

  1. When you select any Work Type except “Hospital, you will be allowed already to input the necessary fields for your experience. After you have inputted the data, just click Add Experience in order to add your experience to the list.

 

  1. However if your Work Type is Hospital, there are some additional fields for you to input. Notice that the fields Company, Ave. Beds, Address and Telephone are disabled because the system requires you to select the Hospital from our database. This is done by clicking on the lens icon which is found below the Work Type.

 

Note:  If you’re currently editing an existing experience, you’ll need to search for hospitals in order to enable the other controls for you to edit such information.

 

 

 

  1. A pop-up window will appear and this is where you search for your hospital that you’ve worked for. Input the criteria for your search and the just click Start Search to find if there is a record of the hospital you are looking for.

 

 

 

  1. If there is no Hospital/s found for your query, just click Input Hospital to manually input the Hospital name, Hospital category, Telephone no., Address and the Bed capacity of that hospital.

 

 

  1. Otherwise, if there is a match with your query, just highlight and click the hospital you wish to select and then click Select Hospital button in order to capture the selected hospital to the previous form.

 

 

  1. Now that you have selected the hospital that you have worked for, you may now continue to input the other information needed.

 

  1. For the Clinical Area, select one from the list. If there is no applicable clinical area for you or if your clinical area is more than one then select Other Experience/s from the list and then input the desired clinical area/s you belong.  Also, input the No. of beds in Clinical Area you’ve selected on the space provided.

 

  1. Select position from the list. If your position is Volunteer Nurse, Nurse Reliever or Nurse Trainee, you must enter the Number Of Hours Rendered Per Week.

 

 

 

  1. If not, just select Others and then input your specific position on the space provided.

 

 

 

 

  1. Meanwhile for Types of patients cared for and Common Procedures Performed, please bear in mind that please put a semi-colon (;) at the end of each item.

 

 

 

  1. When you’re done please be reminded that you must click Add Experience if you are adding a new record or Update Experience if you are currently editing an existing record.

 

 

IV. Non-clinical Skills

 

 

  1. Next is the Non-clinical Skills part. It is in here where we encode other skills which may help us to know you better on the non-clinical aspect. How to access this? Just click Non-clinical Skills in the navigational menu.

 

 

 

 

  1. Adding a new skill has the same procedure as the others above. Just fill up the necessary info and click Add New Skill button. The same goes for editing, just click Edt on the desired record to be edited and then click Update button to save changes or on the other hand press Del if you wish to delete it.

 

 

V. Examinations

 

  1. Subsequently is the Examinations part. It is where you input your examinations passed and taken. How to access this? Just click Examinations in the navigational menu.

 

 

 

  1. Notice that if you select an Exam Type, there are some items that appear and disappear. Just fill in the information needed for you to accomplish such as Dates, Scores etc. By the way, fields such as Date Taken and Date Issued are required.

 

  1. Adding a new examination has the same procedure like the others. Just fill up the necessary info and click Add Certificate button. The same goes for editing, just click Edt on the desired record to be edited and then click Update button to save changes or on the other hand press Del if you wish to delete it.

 

 

VI. Relatives in the USA

 

 

  1. Another part in the Nurse Online Application is the Relatives in the USA. This is where you input the relatives that you have in the USA. How to access this? Just click Relatives in the USA in the navigational menu.

 

 

 

 

  1. Adding a new examination has the same procedure like the others. Just fill up the necessary info and click Add Relative button. The same goes for editing, just click Edt on the desired record to be edited and then click Update button to save changes or on the other hand press Del if you wish to delete it.

 

 

VII. Sources of Application

 

  1. Next part in the Nurse Online Application is the Sources of Application. This is where you input how you learned about the company. How to access this? Just click Sources of Application in the navigational menu.

 

 

 

 

  1. Adding details here is just simple. Just put a mark on the checkboxes that correspond to your answer and fill up the necessary info that relates to it and then click Save button. In editing however, just edit the item/s you want to edit and just click Save to apply changes.

 

 

VIII. Equipment/Tools

 

  1. Another important item in the Nurse Online Application is the Equipment / Tools Checklist. This is where you input the corresponding ratings on how you are knowledgeable with that specific tool / equipment. How to access this? Just click Equipment/Tools in the navigational menu.

 

 

  1. Just input the corresponding ratings on how you are knowledgeable with that specific tool / equipment then click Save Ratings button. Please be reminded to click the Save Ratings button every time you finish inputting your ratings on each category. To edit, just change the rating for the item you want to edit then just click Save Ratings button to save changes.

 

 

IX. Certification

 

  1. Another one is the Certification. This is where you input the corresponding ratings on how you are knowledgeable with that specific tool / equipment. How to access this? Just click Certification in the navigational menu.

 

 

 

 

  1. Adding a new examination has the same procedure like the others. Just fill up the necessary info and click Add button. The same goes for editing, just click Edt on the desired record to be edited and then click Update button to save changes or on the other hand press Del if you wish to delete it.

 

 

X. Petitions / Offers

 

  1. Subsequently is the Pending Petitions and Offers part. It is where you input your pending petitions and offers from foreign and/ or local companies. How to access this? Just click Petitions / Offers in the navigational menu.

 

 

  1. Adding details here is just simple. Just put a mark on the radio buttons that correspond to your answer and fill up the necessary info that relates to it and then click Save button. In editing however, just edit the item/s you want to edit and just click Save to apply changes.

 

 

XI. Restrictions

 

  1. Next is the Restrictions part. It is where you input your last medical examination, its results and other medical conditions you have (if any). How to access this? Just click Restrictions in the navigational menu.

 

 

  1. Adding details here is just simple. Fill up the necessary information that relates to your answer and then click Save button. In editing however, just edit the item/s you want to edit and just click Save to apply changes.

 

 

XII. Personal Documents

 

  1. Last but not the least is the Personal Documents part. It is where you input your identification numbers such as SSS, TIN, Passport #, GSIS and many more. How to access this? Just click Personal Documents in the navigational menu.

 

 

  1. Adding details here is just simple. Fill up the necessary information and then click Save button. In editing however, just edit the item/s you want to edit and just click Save to apply changes.
